My heart goes out to Sorrel and her family. I have two small children and could not imagine losing them, much less to a careless mistake.
I wish them all well. ML

We too experienced a horrible tragic loss of our three month old
    due to a pharmacy error/overdose.

We were unable to settle the case
with any admission of guilt by the hospital, but we did ultimately settle the case out of court.

Instead we had to contact the Ohio State Board of Pharmacy. They found several violations in the Cincinnati area hospital pharmacy related to our son Michael's case.

As parents we must let other parents know the dangers that lurk in US hospitals especially, as they have the honor of having some the highest rates of medical errors in the developed world. Most tragically the rates are especially high in pediatric only hospitals.

We thank Sorrel and her family for telling their story, because it is so eerily familiar to us. We too were strong advocates for our son and despite our questioning of the dosage he was about to receive he still got it. Not once but twice. The hospital did not find the error. We insisted the drug be double checked as some still remained in his cart.

We wish to honor our son Michael's legacy with charitable work. We thank you for sharing your story.
